Waikato special authority requests for methylphenidate and dexamfetamine

Dr Tony Muller, psychiatrist, is able to review special authority (SA) requests for methylphenidate and dexamfetamine, and apply for them if the information supplied confirms the appropriateness of continued treatment.

In the majority of cases Dr Muller can action these without needing a psychiatric assessment.  

Information required from practices

1. A copy of the diagnostic assessment (if this was done privately or outside Waikato DHB – if it was Waikato he should be able to see it on CWS).

2. Confirmation that the patient is benefiting from the treatment. This can be informed with the following questions.

• How often does the patient take the medication?
• What happens if the patient doesn't take the medication?
• Has the patient needed/requested dose increases or reductions?

3. Does the patient have any adverse effects?

4. Are there any concerns about misuse? The patient requesting “lost” prescriptions, or picking up repeats earlier than would be expected might indicate this. Co-morbid substance use is another relevant consideration.

Once Dr Muller has received all information and is satisfied with the appropriateness, he will apply for the special authority and send details via BPAC. Please do not apply for this yourself as the SA application forms part of his documentation for endorsement.
