Tui Ora’s outreach immunisation service (OIS) supports 29 general practices across Taranaki by reaching out to whānau and tamariki who face barriers to accessing scheduled immunisations. Pinnacle is supporting the programme for around two years to help lift childhood immunisation rates.

The Tui Ora outreach immunisation service (OIS) programme provides immunisation support to 29 general practices across Taranaki. The service was implemented to reach whānau and tamariki who face barriers to accessing scheduled immunisations.
Pinnacle is providing support to OIS in Taranaki.
Our Pinnacle team includes Chantel, an experienced childhood immunisation nurse, and Angela, programme support / kaiāwhina.
Together we meet weekly to coordinate and manage Tui Ora outreach immunisation service (OIS) with triage and share general practice OIS referrals. Pinnacle will be supporting this space for approximately two years, with the intent to raise our child immunisation rates.
Once immunisations have been completed, or if a decline is received, AIR is updated and the patient will be discharged back to their enrolled general practice. If there are barriers with access to their general practice, we feedback to the general practice with the intention of a resolution for both parties.
- Please ensure your general practice completes and returns the monthly AIR reconciliation report, to minimise our external follow up with GPs with immunisation status updates.
- Ensure your immunisation team completes the standard recall process (x3 recalls) then refer eligible patients to Outreach via AIR.
