Renewal of special authority numbers: psychiatric medications (Waikato)

PHARMAC requires special authority numbers for some medications to be renewed after a period of time (every 12 months for antipsychotic depot injections risperidone, olanzapine, paliperidone and every 24 months for methylphenidate and dexamphetamine.)

Methylphenidate and dexamphetamine renewals only 

Please send the referral to your PHO psychiatrist (Pinnacle - Dr Tony Muller, Hauraki - Dr Diana Kopua) instead of the Waikato DHB adult mental health triage service and include the following information (where needed we may seek further information to action the referral).  

  • Indicate past diagnosis, including the doctor's name and specialty who made the initial diagnosis and commenced the treatment.
  • Describe evidence of benefit, ongoing need for the medication, and what happens if the patient stops taking the medication.
  • Describe results of monitoring and any side-effects related to the medication.
  • Describe any concerns about abuse or diversion.

New referrals and those requiring diagnostic clarification or presenting with complex co-morbidity should be sent to Waikato DHB triage.
