Co-response team (CRT) redirect

Overview

This service is designed to support a co-response team (CRT) where mental health clinicians, kaimahi Māori, Police and St John will work together when responding to mental health emergency calls.

From time to time there may be a requirement for primary care to provide support for this service, for mild to moderate patients, as identified by the CRT.

When the CRT is supporting a patient who fits the threshold, the clinician in the CRT will make contact with the general practice where the person is enrolled and seek to confirm if a same or next day appointment is available. If no appointments are available, the CRT may also choose to contact an urgent care provider. 

The CRT will confirm the available appointment and determine with the clinician the most appropriate means of sending through any relevant patient details, assessments and management plan. 

This person is likely to require:

  • PRN medication i.e. something for anxiety, sleep or to start antidepressant
  • referral to HIPs and health coaches and primary health services
  • referrals for physical health needs routine testing etc.

The consultation fee will be funded for eligible patients.

CRT will not redirect a patient unless the receiving practice has accepted them. Patients will be provided with information regarding the appointment that has been made for them.

For conditions that meet Primary Options Acute Care criteria, practices can initiate a separate referral and claim via the usual Primary Options Acute Care process.
