Primary mental health brief intervention, counselling and psychology services (referral)

Overview

The Primary Mental Health Brief Intervention Service provides support for individuals experiencing mild to moderate mental health issues. 

Taranaki Primary Connections is not an acute or crisis service and is therefore not the best option for individuals requiring urgent assistance.

This is referral guideline ONLY.  Practices refer patients to the service which is provided by a contracted third party provider.

ICCT – physical and transition of care pathway for patients with long term mental illness

The service focuses on collaboration between the patient, their GP and mental health providers to remove barriers for these patients to access primary care, and to assist the patient to reach a stage where they are able to self-fund their primary care.
